Open Enrollment for the 2018/2019 Plan Year will run from May 1, 2018 through 5:00pm on Thursday, May 31, 2018. All contracted and permanent full-time employees of PUHSD will make their benefit elections through the IVisions Web Portal. For security purposes, the IVisions Web Portal can only be accessed via a District networked computer; please plan accordingly. Changes for the 2018/2019 Plan Year
Domestic Partner (DP) Coverage - DPs and the child(ren) of DPs are now eligible for medical, dental & vision coverage. Apply on-line and submit required Affidavit if applicable. See below.
Medical Tier Structure - The new tiers are Employee Only, Employee + Spouse, Employee + Chid(ren) & Employee + Family (Spouse & Child(ren). For tier purposes, DPs = Spouse. Make sure you and your dependents, if applicable, are enrolled in the correct medical tier.
Alliance Plan Option - The Alliance Plan Option follows the same plan design as our current Low Option except the In-Network network is smaller. For more information on The Alliance Network and other medical insurance changes, review the Blue Cross Blue Shield of AZ Updates page.

Domestic Partner Coverage
Effective July 1, 2018, the District will expand eligible dependents to include domestic partners and the child(ren) of domestic partners for our medical, dental and vision benefits only. For medical tier purposes, DPs equate spouse.
In order to add your domestic partner (DP) and/or DP children to your plan(s), you will need to do two things:
1. Please complete your Open Enrollment (OE) between 5/1/2018-5/31/2018 adding your DP and/or DP children to your plan(s). Please make sure that you choose the “Domestic Partner” relationship for your DP on the Dependent Enrollment page; no special designation needed for children. Note: dependents are listed at the beginning of the OE process and then linked to the plan on the applicable insurance coverage page.
2. Please complete the Domestic Partner Affidavit and return to my attention @ CEE-5th Floor prior to the close of OE on 5/31/2018; please see below link. The District will be utilizing the standard BCBS affidavit for dental and vision as well as for medical. The affidavit will be required on an annual basis and must be notarized. Note: it is your responsibility to submit the affidavit if applicable; DP elections without the supporting affidavit can not be processed and DP coverage will be reflected as cancelled for the upcoming year.
Please note: IRS regulations require insurance premiums including DPs to be deducted from employee POST-TAX.

Follow-up RequirementsShort Term Disability = Form
In addition to making your insurance elections online, Short Term Disability (STD) requires you to complete a STD enrollment form if you are new or making changes. The enrollment process is not finalized until both steps are complete. Please turn in the form to Employee Benefits (CEE 5th Floor or Fax#602-274-0484) no later than 5/31/2018 or your application will be disqualified if a new enrollee or returned to your 17/18 level if a current participant.
Note: The full STD form is only required for new enrollments and increases of more than one level. Increases of one level, decreases and cancellations only require submission of the first page of the form. Also, submission of the STD enrollment form does not guarantee coverage for levels requiring medical underwriting for approval.Supplemental Life Insurance = EOIIf you are a new applicant, or you are increasing your and/or your spouse's current election by more than $10,000 for a maximum election of $50,000 or increasing your and/or your spouse's current election amount by any amount over $50,000, please make sure to complete the Evidence of Insurability (EOI) questionnaire online @ The Standard.
